Cerebrolysin

A registered medicine in dozens of countries, derived from pig brain, and the one product here where a purity percentage is close to meaningless.

The short answer

Cerebrolysin is the odd one out in this catalogue in a way that matters practically. It is not a synthetic peptide of known sequence but a standardised enzymatic breakdown of porcine brain tissue, a mixture of low molecular weight peptides and amino acids. It has genuine registration in around 44 countries and decades of clinical use for stroke, brain injury and dementia. Western evidence assessments have been considerably more equivocal than that footprint suggests, and it has never been approved by the FDA or centrally by the EMA. The practical consequence for a buyer is unusual: because the product is a mixture, an HPLC purity figure of 98% or 99% cannot describe it in the way it describes a single peptide, and an animal tissue origin raises identity and contamination questions that synthesis does not.

Regulatory statusApproved in roughly 44 countries and in neither of the two that matter most for this site's readers. Cerebrolysin is a registered prescription pharmaceutical across Russia, China, Korea, Mexico and much of Eastern Europe including Romania, Poland, Ukraine, Hungary and Bulgaria, with indications typically covering ischaemic stroke, traumatic brain injury, vascular dementia and dementia of the Alzheimer type. It is not FDA approved and it is not centrally authorised by the EMA.
Compounding statusCerebrolysin cannot meaningfully be compounded, because it is not a synthetic peptide. It is a standardised preparation derived from porcine brain tissue, and that manufacturing origin is the single most important thing to understand about it.
Anti-dopingNot specifically named on the prohibited list.

What it is

Cerebrolysin is produced by controlled enzymatic breakdown of purified porcine brain proteins, yielding a mixture of low molecular weight neuropeptides and free amino acids. It is administered as a solution by injection or infusion. The claimed mechanism is neurotrophic, described as mimicking the action of endogenous growth factors, which is a mechanism attributed to a mixture rather than to an identified active molecule.

Who it is for, and who actually takes it

In the countries where it is registered, the approved populations are patients with ischaemic stroke, traumatic brain injury, vascular dementia and Alzheimer type dementia, and in some markets children with perinatal brain injury. These are supervised clinical settings.

It is bought by healthy adults as a nootropic, for cognition, recovery and neuroprotection, entirely outside any registered indication and usually without any awareness of what the preparation actually is.

What the research shows

The trial literature is large, heterogeneous and independently assessed as equivocal. Selecting one trial to chart would misrepresent a body of evidence whose defining feature is inconsistency.

RouteIntramuscular injection or intravenous infusion, in clinical settings
FormSolution, not a lyophilised single peptide

We record the registered indications and route rather than a schedule, because the registrations are national with differing labelling and there is no single published label to reproduce. Note that the registered product is a solution given in clinical settings, frequently by infusion, which is not what a research vial provides. We publish no reconstitution guidance.

What happens when you come off it

In the registered indications, treatment is given as defined courses in acute and rehabilitation settings rather than continuously, so discontinuation is built into the intended use. There is no controlled evidence about what happens when healthy users taking it as a nootropic stop.

What it is being studied for

AreaStage of evidence
Ischaemic strokeRegistered indication in many countries, Western evidence assessments equivocal
Traumatic brain injuryRegistered indication in many countries
Vascular dementiaRegistered indication in many countries
Alzheimer type dementiaRegistered indication in many countries, Western assessments equivocal
Cognitive enhancement in healthy adultsNo approved indication and no controlled evidence

There is a substantial trial literature and independent systematic assessment has repeatedly concluded that the evidence is weaker than the breadth of registration implies. A wide regulatory footprint and a convincing evidence base are not the same thing, and here they diverge.

Common myths

It is approved in 44 countries, so it must work.

Breadth of registration reflects regulatory history and regional practice as much as evidence strength. Independent systematic reviews have been considerably more cautious than the registration footprint suggests, and neither the FDA nor a central EMA authorisation is among those 44.

It is a peptide like the others on this site.

It is not. It is an enzymatic hydrolysate of porcine brain tissue, a mixture of many peptides and amino acids rather than one synthesised molecule of known sequence. That difference changes what testing can tell you about it.

A 98% purity certificate confirms the product.

Purity of what? The product is a mixture by design. A single purity percentage cannot characterise it the way it characterises a synthetic peptide, and a certificate presenting one should make you ask what exactly was measured.

Animal derived means natural and therefore safer.

Animal tissue origin introduces contamination and immunogenicity considerations that chemical synthesis avoids entirely. It is a reason for more scrutiny of the manufacturing chain, not less.

Verifying a purchase

Apply different thinking here. For a synthetic peptide you confirm one molecule by mass spectrometry. Cerebrolysin is a defined mixture derived from animal tissue, so the questions that matter are about the manufacturing process, batch to batch consistency of the preparation, and the controls applied to the biological starting material. A high purity percentage on a mixture is not an answer to any of those questions, and if the documentation offers only that, it has not told you what you need to know.

The general method is set out in our guide to reading a certificate of analysis, which covers the eleven checks that apply to any compound. The points above are the ones specific to Cerebrolysin.

Caveats and safety

Porcine tissue origin carries considerations that synthetic peptides do not, including immunogenicity and the integrity of the biological supply chain. The registered product is given under clinical supervision, frequently by infusion, in patients with acute neurological injury. Independent evidence assessments of benefit have been equivocal, which means the risk side of the calculation is being weighed against a benefit that is itself uncertain.

Scope. This is an evidence review, not medical advice. Where an approved label schedule is reproduced it is a record of what a regulator has published about a licensed medicine, not a recommendation and not a schedule for anyone to follow unsupervised. For unapproved compounds we report only the doses used in published studies, attributed to those studies, and we publish no protocols and no reconstitution guidance for any compound. Regulatory status differs between countries and changes over time. Material sold for laboratory research is not approved for human use. Decisions about treatment belong with a qualified prescriber.
